RANDOM DRUG TESTING POLICY AND ALCOHOL TESTING
In compliance with employment, and as required in contractual agreements with clients of Provide A Nurse, all employees of Provide A Nurse are required to submit to drug/alcohol testing prior to employment and random drug/alcohol testing. If prior to an assignment a client requests an updated random drug test, the employee is required to submit to testing within twenty-four hours as a condition of employment with Provide A Nurse. Immediate drug and alcohol testing may be required during a shift and failure to do so is grounds for dismissal.
Refusal to complete the drug testing or a positive test for an illicit drug/alcohol or unprescribed medication use will result in immediate dismissal.
Upon presentation of a random drug/alcohol testing form, the employee will review this policy sign and date the policy as a condition of employment. Refusal to abide by this policy and sign this form is grounds for dismissal.
